Stromal vascular fraction (SVF)-enriched autologous fat grafting (AFG) resulted in greater long-term graft retention than standard AFG in patients undergoing aesthetic and reconstructive breast procedures, according to results of a prospective, multicenter randomized study published online in Aesthetic Surgery Journal.
Investigators conducted the study across 6 centers between January 2021 and March 2025, and they randomized 312 patients, with 156 assigned to standard AFG and 156 to SVF-enriched AFG. Participants had bilateral breast hypoplasia, breast asymmetry associated with unilateral breast hypoplasia, breast ptosis or pseudoptosis, or post-lumpectomy defects.
MRI Shows Greater Graft Retention With SVF-Enriched Fat Grafting
The primary endpoint was MRI-based graft retention at 12 months, assessed using 3-dimensional segmentation protocols and analyzed by breast radiologists. Secondary outcomes included 24-month retention, complications, BREAST-Q outcomes, radiologic findings, and revision procedures. Mean follow-up was 28.6 ± 6.4 months.
At 12 months, mean graft retention reached 71.8% ± 11.2% in the SVF-enriched group compared with 56.4% ± 13.7% in the standard AFG group (P < .001). The difference remained at 24 months, when retention was 66.2% ± 12.0% versus 49.8% ± 14.1%, respectively (P < .001).
BREAST-Q scores were higher in the SVF-enriched group at both follow-up intervals, although the abstract does not provide numerical scores. Rates of fat necrosis and oil cyst formation were similar between groups, and investigators reported no suspicious MRI findings attributable to fat grafting during follow-up.
“SVF-enriched AFG demonstrated improved volumetric persistence and higher patient-reported satisfaction compared with standard AFG, without increased radiologic or clinical complications,” the authors wrote. “MRI-based evaluation may improve reproducibility in future breast fat grafting studies.”
