In each issue, Modern Aesthetics® spotlights a new device that cosmetic physicians are introducing to their patients. Here, Arash Akhavan, MD, a New York City-based dermatologist, discusses Revelle Aesthetic’s Avéli, the latest cellulite treatment.

What is Avéli?

Arash Akhavan, MD: Avéli is a US Food and Drug Administration-cleared device that delivers a reduction in cellulite in both the thighs and buttocks after a single, minimally invasive, in-office procedure. Treatment takes about an hour and has little downtime for patients. During the procedure, I guide the Avéli device under the skin directly to the targeted septa, which are a major underlying cause of cellulite dimples. I deploy a hook to identify which septa under the skin are the culprit septa, then expose a sharp edge to cut through and release those septa, allowing the skin to bounce back and smooth out.

How does Aveli compare to other cellulite devices?

Dr. Akhavan: Many other cellulite products and treatments try to treat cellulite from the outside in. However, because the fibrous septa networks under the skin are so complicated, it’s important to consider those when treating cellulite. Avéli addresses cellulite from the inside out. It’s the only device that allows providers to identify which septa under the skin are causing a cellulite dimple, and then confirm in real time that I am releasing those targeted septa. This gives me more confidence that I am treating what I intended to treat.

Dr. Akhavan: As with many cosmetic procedures, patient selection is key when it comes to Avéli. We stress that the procedure only treats cellulite dimples and not loose skin or ripples in the skin. We also think it is important to select candidates that are healthy with good skin firmness and elasticity allowing the skin to effectively bounce back.

What types of results are patients seeing with Avéli?

Dr. Akhavan: I’ve seen highly positive results so far. Patients are finding the treatment to be easy with strong efficacy, and they have resumed their normal activities with minimal or even no downtime. Patients will start to see initial results within days of the procedure and continue to see a reduction in the appearance of their cellulite as any post-procedure swelling or bruising dissipates. Cellulite is such a personal and emotional issue for so many of the patients we treat. It has been touching to see tears of joy when patients first witness their before and after images. Patients have been so happy that the whole office gets very excited to treat the Avéli patients on our schedule.

Is treatment with Avéli painful?

Dr. Akhavan The procedure is completed after the treatment area is numbed with a local anesthetic and is well tolerated by patients. In the clinical study, the most common side effects reported were mild pain within the first 24 hours and bruising and tenderness to the touch, which typically resolved within 30 days. Small areas of firmness, usually not visible or painful, typically resolved in a couple of months.

How long do Avéli results last?

Dr. Akhavan: Avéli delivers a long-term reduction in the appearance of cellulite after just one treatment. Patients in the clinical study were followed for a year after their procedure and demonstrated lasting results at their 12-month follow-up. Meaningful results are visible quickly after the procedure once swelling or bruising subsides.
