In each edition, Modern Aesthetics® asks top aesthetic physicans about the newest devices in their practices. This month, BABAK AZIZZADEH, MD, FACS talks about the FRAXEL DUAL LASER SYSTEM FROM SOLTA MEDICAL.

WHAT IS THE FRAXEL DUAL LASER SYSTEM? HOW ARE YOU USING IT?

Earlier this year, the Fraxel Dual 1550/1927 laser system received FDA approval for the treatment of pigmented lesions such as lentigos, solar lentigos, and ephelides. It is also indicated for use in dermatological procedures requiring the coagulation of soft tissue, as well as for skin resurfacing procedures. Specific additional indications include actinic keratosis (AKs), melasma, treatment of periorbital wrinkles, acne scars, and surgical scars.

The system penetrates the skin down to the dermis without disrupting the skin's protective outer barrier, maximizing safety and recovery time and resulting in natural stimulation of new collagen creation.

“Our Fraxel Dual device has two different wavelengths available for use—the 1550nm and the 1927nm. Each of these wavelengths can be used on their own or as a combination treatment,” says Dr. Azizzadeh, explaining that both wavelengths are using fractionated non-ablative technology. He says the 1550nm is used for deep collagen remodeling or to improve texture and that the 1927nm is also highly effective for removal of sunspots and melasma.

“The chromophore that the Fraxel laser targets is water, which makes it much more effective than other lasers that target only pigment. We have found the 1927 to be much more effective than the IPL technology,” says Dr. Azizzadeh. “When used in combination the patient gets the best of both, improving texture and removing pigment.”

HOW DOES THE FRAXEL DEVICE FIT INTO YOUR PRACTICE?

Dr. Azizzadeh says that in his practice they were previously using another laser for improving texture and removing pigment, but have noticed better results with the Fraxel Dual laser.

“We also notice that it is a great adjunct treatment for any surgical procedure for full facial rejuvenation,” he says.

He says he has had a positive reaction from patients and seen an uptick in return business as well as in referrals as a result of using the device in his practice.

“Our patients are really pleased with the results. We are getting a significant amount of return business as well as referrals from current patients.”

WHAT BENEFITS HAVE YOU SEEN SINCE INCORPORATING THE DEVICE? ARE THERE ANY DRAWBACKS?

Dr. Azizzadeh notes that incorporating the Fraxel Dual system into his practices has allowed him to attract new patients, treat more patients, offer new treatments, and increase revenue.

“First of all, the brand name is well known, which helps, but the ultimate factor is the results. Word of mouth is the best form of advertisement, therefore when patients are getting good results they are talking about it to everyone,” he explains. He says the device is easy to use and gives great results with little downtime. He says there are no real drawbacks— although there are consumables, he says they are very affordable.

He does note that if a patient wants a one-time treatment, this is not the device for them because it does take four to six treatments to get desired results with the Fraxel Dual laser.

“If they do want a one-time treatment, we usually utilize the DEKA ablative fractional CO2 laser. This laser is a more intense treatment. Depending on the condition being treated, one to two treatments are recommended. The healing time is about five to 10 days. During this time the patient will experience swelling, pinpoint bleeding and crusting. Although the healing time is longer than with the Fraxel laser, the results are better after only one to two treatments,” he explains. “Both lasers give similar results. It is really up to the patient how they would like to conduct the treatments. Either more treatments with less down time, or less treatments with more down time.”
